We have cooperative arrangements with many major insurance carriers including Medicare. You will be asked to provide us with all necessary updated insurance information at the time of your initial consultation. Please check with the office for specific insurance carrier information. To determine if your surgery will be covered by your insurance company we will file a pre-authorization with your carrier. This will help you to determine your out of pocket expenses prior to your surgery. You will be responsible for the difference between the covered amount and our fee, as well as for any deductibles or co-payments. We will be happy to prepare a claim form that will be mailed to you or your insurance company. Our billing specialist can meet with you to discuss specifics, or you may call her directly at 269-372-3000, ext 111 Cosmetic Surgery / Insurance Cosmetic procedures are not normally covered by insurance. At the time of your consultation you will meet with the surgery coordinator to discuss your fees. She will itemize all fees including surgeon, hospital and anesthesia fees. Should you elect to proceed with your cosmetic procedure, your initial consultation fee will be deducted from your surgeons' fee. All cosmetic surgery fees are due in the office one week prior to your surgery. We accept cash, check or major credit cards. For the convenience of our patients we also offer patient financing. Our surgery coordinator will be happy to help with any financing questions. We strive to provide you with all the information you need to make an informed decision. We have standard charges for most procedures and will gladly answer your questions. Prepayment is customary for cosmetic surgery. At your consultation, we will provide you with all the information you need to make an informed decision about how to pay for your surgery. We accept cash, checks or major credit cards. We offer affordable financing for your convenience.
